General information

If you consider that, your personal data is being processed in violation of the law of Georgia “On Personal Data Protection”, you can address the Service via an application form and ask for an examination on the lawfulness of your personal data processing.

 

The application form should include the following information:

• The name, surname, current residence (where a delivery of correspondence will be possible) and/or an E-mail address of applicant and his/her representative (if applicable), if the applicant prefers to send the correspondence via an email, telephone number and/or other contact information. In case of an application by a representative, providing only the contact information of the applicant and his/her representative is sufficient;
• Identifiable information regarding the controller and/or processor allegedly infringing the applicant’s rights under the law, making it possible to identify the controller/processor without a disproportionate effort;
• Possible date of the alleged offence (day/month/year);
• Description of the circumstances and meaning of the alleged offence;
• The data subject’s (applicant) request;
• Submission date of the application;
• Signature of the applicant or their representative.

In addition, the application should include a copy of the applicant’s ID card as an attachment (excluding instances where the application is signed and/or stamped digitally according to the law of Georgia “On Electronic Documents and Electronic Trust Services”) and, if available, appropriate evidences (e.g., photographs).

In case of an application with the use of a representative, the application should also include relevant documents confirming the representative’s power, copy of the applicant and representatives’ ID cards, but if the application is signed and/or stamped digitally according to the law of Georgia “On Electronic Documents and Electronic Trust Services” presenting the signer’s ID document won't be mandatory.

Application is possible in writing (Personal Data Protection Service (address.: Tbilisi, Vachnadze St. №7; Batumi, Bako St. № 48)) as well as digitally (signed and scanned or in a photo form). If the application is sent digitally, it should be sent through the Personal Data Protection Service’s case management program or to the Service’s official E-mail addresses – office@pdps.ge.

We further remind you that filling in the standard application form developed by the Service is not mandatory, most importantly the application submitted to the Service should contain the above-mentioned information in full.
